Personal Support Services

Program Overview

Anchor Home provides personal support services for adults who chose to live in their own home or apartment or the home or apartment of a family member. The Personal Supports program aims to enhance the quality of life and promote independence for participants by offering personalized care, tailored to meet their unique needs and preferences.

The primary objectives of the Personal Support Services program are as follows:

  1. Empowerment: Enable individuals with disabilities to lead fulfilling lives by promoting self-determination and empowerment, allowing them to make decisions that affect their daily routines and lifestyle choices.

  2. Independence: Foster greater independence among participants through skill-building activities, training, and support, thereby increasing their ability to manage everyday tasks and challenges.

  3. Personalized Care: Provide individualized support services that cater to the specific needs, goals, and interests of each participant, recognizing that each person's journey is unique.

  4. Community Integration: Encourage social integration and active participation within the community, promoting inclusion, social relationships, and meaningful engagement in various community-based activities.

  5. Family Involvement: Recognize the vital role of families in supporting individuals with disabilities and collaborate with them to develop person-centered plans and implement effective support strategies.

Program Services:

The Personal Support Services program encompasses a wide range of services designed to address various aspects of daily living and overall well-being. These services include, but are not limited to:

  1. Personal Care Assistance: Trained support professionals assist with activities of daily living (ADLs) such as bathing, dressing, grooming, meal preparation, and medication reminders.

  2. Life Skills Development: Participants receive coaching and training to enhance their social, communication, and daily living skills, enabling them to achieve a higher level of independence.

  3. Community Access: Support is provided to encourage and facilitate community integration through participation in recreational, educational, and cultural activities, fostering a sense of belonging and social connectedness.

  4. Transportation: Accessible transportation services are provided to enable participants to travel to appointments, work, and community events, promoting greater independence and mobility.

  5. Behavioral Support: Specialized behavioral support plans are followed and implemented to address challenging behaviors and promote positive coping mechanisms.

Eligibility Criteria:

To qualify for the Personal Support Services program, individuals must meet the eligibility requirements set forth by the Maryland Developmental Disabilities Administration (DDA) through the Community Pathways Waiver.
